This is a simple thing to diagnose, tripod and a book, shoot the book with tripod and timed release or cable, if the images are sharp, hand hold same shot same settings if they are out of focus its you. Do this first in manual focus, then repeat through the focus settings, 10 mins alter you will know if it's your kit or not.
