Hi Braveboy, yeah the sun was high in the sky and it was an awful time to be shooting anything so I'm actually amazed that anything came out reasonable at all... :D
I love both shots... But the lighting in shot #1 is wonderful, the whole contrast range through the shot sets a deep, dark mood but at the same time picking out the skin flaws and generating depth. I love it!
I still have the lamp and those bits are still as dusty today. I didn't want to Photoshop the dust, I wanted a pure image which for the most part is what it is (just a crop and some vibrance I think).
I think you're right Tom, when I started (so many years ago) we always referred to Depth of Field and I don't remember using the word Bokeh at all. It seems now seem to be a cover-all word for almost anything Depth of Field related... I still call a lens a lens and not "glass", in my opinion...

We have a Lastolite Hilite background with 2 studio heads inside and two more studio heads to illuminate the dogs. The dogs sit (some of the time) on a table with a white canvas on it. In all cases there are pretty much as shot but we do remove some harsh shadows when they happen.
I did clip the ear when the shot was taken, based on the comments above, here are my re-crops....
