I'd guess its not the card - the file appears to have been corrupted after you copied it to your PC. The reason I think this is because the explorer preview is fine which would suggest the file was fine when that was generated.

Can you check how many lrcat files you have? When you first ran the reinstalled copy of lightroom it would have created a new one and if you then imported your original files into the new folder they would all have no edits on them. Is there perhaps your old catalogue somewhere...

Does the lens have a focus limiter that has been accidently set on? Some lens have this to limit the focus range when desired to stop too much focus hunting. Can be a bit confusing and worrying when you don't realise its been set.
Lightroom 4 has the map module that allows you to do this.
