Canon 580ex - ignoring eTTL?

I'm not sure if I changed a setting but last night I had problems using a 580ex on a 5D2.

Camera was in manual, flash was in eTTL.

In the first photo all is OK. In the second photo the flash fired in full power and seemed to ignore eTTL. Looking at the EXIF data, in the "bad" image is says "Flash Off, Did not fire". Any ideas or suggestions what's happening?

I had a similar problem with my new 430EXII, until I realised that I hadn't pushed it into the hot shoe far enough before I locked it.
