Yes. Basic processing in Lr (though it could be done just the same in ACR) to get a reasonably flat starting point. Blended in a darker exposure for the highlights using luminosity masking. Then it's a matter of painting in some light (basically enhancing where the light is already hitting), some selective vignetting, and a straightforward Orton effect.
