this my sound like a stupid answer, but i'm guessing apple do repairs. best to take it to an apple store for advice.
 
If you have already had the back off can't you take it off again and see what you have disconnected/broken. It shouldn't be too much of a mystery.
 
earpiece is one of the 3 wires for the screen. Had the same problem with my wife's when I did the screen for her, just popped the screen up again, reconnected all 3 wires, and then all good. Not sure about vibrate though.

Before you send it off again, pop it back open and check the wires. It will probably be a lot simpler than you thought
