Lightroom settings for Intuos4

Does anyone want to share their setup for using the Intuos4 with Lightroom?

Just moved from PC to Mac and can't transfer the settings I was using, so thought it was a good time to see how other people are using it.

The only one I've found online is only downloadable by NAPP members.
