OK, not miniatures.... Whilst searching for Tilt/Shift lenses I found this interesting website called TiltShiftMaker, any way these were a couple I tried and liked.

For the effect to work properly, everything on one vertical plane has to have the same amount of lens blur. These look like they've just had a letterbox of clarity retained. For example, in the first image, the bridge columns suddenly go from in focus to out-of-focus. This isn't realistic and betrays the illusion somewhat.
The same thing is going on in the second but it's slightly less obvious.
 
Obvious in the second too... that fence should all be on one focal plane... or close to it anyway, as should the lamp posts.

You really need to do this yourself in Photoshop. No automated process will ever get it right. It can't... it's actually impossible for an automated process to get this right if you think about it.
