Lots of ways to do it but pretty much layer them all, align them all and mask parts of each frame. Auto align speeds up the whole process now.
Dan Carr made a decent video explanation here: http://vimeo.com/2526445 but I think he says to delete a lot of content then align. Better to align first with auto align, then delete most of the frame you don't need (circle round the subject), then mask the specific detail.
