Recommend a Monopod Ball Head

I'm looking for a monopod ball head s that I can do this...

Currently have a Giottos monopod and a Nikon 200-400 to go on top.

I just got a Redsnapper RSH-61 to use on my Gitzo Traveller tripod. Although it's pretty light I reckon it might work. The head they supply with their monopod is also good, and larger, but I'm not sure if they sell it other than with the monopod. The RSH-61 is £29.99 complete with plate and 10% off for TP members.
 
I've just switched from a ball head to one of the Manfrotto pistol grip heads. Works great for me.

Is that with a big lens on a beltpod/monopod?

I use a Redsnapper RSH-12 on both my tripod and monopod (same one - I swap it across) and it works very well on both with a Sigma 150-500mm. I've never supported the monopod from my belt though! I often extend the monopod out with the stick sloping to the rear at the bottom and 'brace' the bottom of the stick against the instep of my rear foot - so the way I hold the 'rig' is similar to the photo you posted. I've been thinking about getting the smaller RSH-61 that magirus has so that I can keep it on the monopod. Both the Redsnapper heads use 'Arca Swiss' type plates.

I think in fact I'm only after a tilt head, not a ball.

The only question is, which tilt head, one which will support the weight (around 4.5kg) of the lens and camera....?

Any ideaS?
