Tilt Shift?

After looking at some of the photo's elsewhere on the site, I thought I'd best ask the question - what is Tilt Shift? I quite like the way it gives the subject an almost toy model look, but how is it achieved?

Either with a lens that allows you to change its alignment with the sensor, or by software that provides a similar effect in post production.

It changes the plane of focus buy "tilting" the lens.
