Has anyone ever seen this before?

Is it constant or was that a one off? I've seen things like this on Macbook Pros and G5 desktop machines. There's even a flickr group called Glitch Art for this kind of stuff.
 
Looks like it could be overheating IMO

have you tried to clean the fan on GPU ? As Marcel said it can be overheating,more likely the chip then memory but not sure.try to clean the fan and try then.
If this wont help you will have to get new gpu...
 
Sadly dead card mate. Mine died middle of last year (also a 9800) in my G5. 5 years of service. And its still quicker than my macbooks!

New macpro has been sanctioned by purchasing (wife) Im just waiting on Cupertino to release the new models.
