parental consent forms

[CarlukeDave]

I'm working at the launch of a new toy store tomorrow night, the brief from the customer includes photographs of children inthe store. When doing stuff like this in the past, the client has sorted out the parental consent for the use of the photographs of the children. This event is being coordinated by a PR company and they are looking for me to provide forms.

Could anyone advise if there is anywhere I can pick up a template or generic form?

Many Thanks

Dave
