Questionnaire form for site

Hi guys,

Not photo related but I hope someone can help...

I would like to add a questionnaire on my site so that guests can quickly answer a few questions about their visit to our villa.

I would like the response to be sent as an email to me, what is the easiest way of setting up a questionnaire without getting a load of spam?

I use Dreamweaver to design the site and I haven't got much experience in using forms.

I have just looked at kontactr and it looks great for a simple contact us form but you can not do a bespoke questionnaire on it

Cant you? Im sure I did one. Maybe they changed it..

you could use a google docs form, it will email you when someone has entered something and store the answers in a spreadsheet.
