Are you using the match needle meter or a hand held device?
I use a handheld meter (a Sekonic L-308) and use incident readings most of the time.

I've only tried the camera's meter once and discovered it only meters up to 400asa (and I had some pushed HP4+ in the camera at the time). I do mean to try it at some point though as it might make things faster in some cases. I've seen people say it's not very accurate, but I guess the film I usually shoot will have sufficient latitude to avoid major problems.
 
No, but not far from it. It's maybe a quarter mile behind where I took this shot.
